I downloaded the PDF version of the form but I can't open/view it?

All of the forms in this website are provided in two different formats for your downloading and viewing convenience. If you do not have the Free PDF viewer from Adobe you can download it here for free.

Why do I have to scroll side to side to view the whole page?

This website is designed to be viewed at a monitor resolution of 800x600 or higher. If you are having to scroll side to side to view the site your monitors resolution is very likely set to 640x480.

To correct this issue, do the following:

  1. From your desktop right click
  2. Select Properties.
  3. In the window that open there are tabs across the top, select the Settings tab.
  4. In the lower right corner is a slider. Move the slider to the right. You will see the numbers below it change.
  5. Move the slider to 800 by 600 or 1024 by 768.
  6. Select apply at the bottom of the window, then select OK.

You might want to try some different settings to find the one that is best for you.
Please Note: Some older computers are not able to support screen resolutions higher that 640x480. There are to many reason for us to list here. Please refer to your owners manual should you continue to have this problem.

Why is the text so large?

This website is designed to be viewed at a monitor resolution of 800x600 or higher. Text is sometimes displayed larger that intended for several reasons, here are two of them:

  • It is possible that your screen resolutions is set below the minimum viewing standard for this website. Please refer to the above solution to check your resolution.
  • Many time people with visual impairments have the font sized increased beyond Normal to assist in viewing the contents of their computer.

To check and correct this issue do the following:

  1. From your desktop right click
  2. Select Properties.
  3. In the window that open there are tabs across the top, select the Settings tab.
  4. In the bottom right corner select the Advanced button.
  5. At the top of the window that open your font settings will be display.
  6. Adjust to suit
  7. Select apply, then select Ok
  8. Select OK again to close the Properties window
